Get Ready to Sign Up for Health Insurance

Sign-up is faster and easier when you’re prepared. Have as much of this information as you can for everyone in your household signing up. Your information will stay private. Low cost medical insurance.

Even if you don’t have all of this information, you can still start the sign-up process.

Social Security numbers

Current employment and income information, such as tax statements, W-2 forms or pay stubs

Insurance cards, if members of your household have health insurance

Immigration Documents (required if you are a documented immigrant)

Items that show your legal immigration status in the U.S. can include:

Malpractice insurance

Permanent Resident Card (Green Card/ I-551)

Employment Authorization Card (I-766)

Naturalization Certificate

Arrival/Departure Record (I-94 or I-94A)

ORR Verification of Release Form (ORR UAC/R-1)

Pick a plan that is right for you and your family:

Life insurance protection

Know your family’s health history, including any medications that you take and doctors you see.

If you have a regular health care provider, write down the insurance plan(s) they accept. Call your provider’s office if you are unsure.)

Low- or no-cost health insurance through the New York State of Health Marketplace


Medicaid is a health insurance program for low income individuals, families and children. You may be eligible for Medicaid if you have high medical bills, if you meet the financial requirements or if you receive Supplemental Security Income (SSI).

People who are 65 years and older and those who are, living with a disability or are visually impaired can also apply for Medicaid year-round. Visit Facilitated Enrollment for the Aged, Blind and Disabled.

Child Health Plus (CHP)

Your child may be eligible for CHP depending on your family income. CHP is a health insurance program for children under the age of 19. There are no immigration requirements for CHP. Enrollment is available year-round through the NY State of Health Marketplace. Visit Get Covered NYC, or call 311.

The Essential Plan

The Essential Plan (EP) is a new health insurance program for low income individuals who are not eligible for Medicaid or Child Health Plus. You may be eligible for the EP for as little as $0 or $20 per month depending on your family income. Enrollment is available year-round through the NY State of Health Marketplace. Qualified Health Plan

Qualified Health Plans (QHPs) are private health insurance plans available on the New York State of Health Marketplace. You may be eligible for financial assistance to pay for a QHP. You can enroll in a QHP during the Open Enrollment Period or a Special Enrollment Period. The open enrollment period ended January 31. If you have a qualifying life event such as a change in your family or income or the loss of health insurance, you may be eligible for a special enrollment period. New York City's Early Intervention Program

Do you have a child in New York City's Early Intervention Program? Your child may qualify for health insurance and other benefits even if you already have health coverage. Get more information about health benefits available to children in Early Intervention.


BigAppleRx is the City of New York’s official prescription discount card. You can use the free BigAppleRx card to get discounts on nearly all FDA-approved prescription medications at more than 2,000 NYC pharmacies. You can also save on over-the-counter medications such as smoking cessation aids and diabetic supplies as long as you have a valid prescription. Everyone is eligible to get the BigAppleRx card. You do not need health insurance or do not need to apply for it.
