Once your application and underwriting are complete, Protective will notify you. You’ll receive all documentation electronically — file it away with your other important documents and notify your beneficiaries. Please keep in mind that depending on your medical history and exam results, medical records or additional underwriting requirements may be required. This process generally takes a few weeks.

Why have life insurance

Premium based on the first 5 years of a Costco Gold Star or Business Membership, 20-year period, $250,000 face value for an eligible 40-year-old male at the select preferred non-tobacco rate. Monthly premiums for the Member Advantage Life insurance policy will increase at year six. Face amount listed is for the initial level benefit period. The level premium period, which begins in year 6, may be extended beyond the initial level benefit period, however the face amount will begin to decrease annually until it reaches $10,000, at which point premiums will begin to increase. Actual results will vary based on a variety of factors including age, gender, health and other underwriting factors. Rates are effective as of January 2, 2018.

Coverage increases may require additional underwriting and decreases can be subject to minimums and charges after the third policy year.

Coverage amounts and express application processing may be available to applicants from ages 18 - 45 for up to $1,000,000 and 46 - 60 for up to $500,000 of coverage that meet certain risk and eligibility requirements of Protective Life. Full medical underwriting may be required for applicants that do not meet Protective’s eligibility criteria.
